Persons who wish to provide feedback on the way Catholic Family Services of Toronto provides services to persons with disabilities can use the following methods: click the Accessible Services Feedback Form link on the Agency’s website, www.cfstoronto.com in the Resources section click Accessibility for Ontarians with Disabilities Act and you can access an Accessible Services Feedback Form at the bottom of the page or the receptionist can provide an Accessible Services Feedback Form and envelope. All feedback forms will be directed to the Human Resources Manager. Persons submitting an Accessible Services Feedback Form can expect to hear back within ten (10) business days, if appropriate.

Complaints about services to persons with disabilities will be addressed according to our organization’s complaint management procedures. For more information our “Guidelines and Procedures for Concerns and Complaints from Clients and Wellness Group Participants” brochure is available from the receptionist or on-line at www.cfstoronto.com in the Resources, Agency Documents section.

The Agency also employs a Client Feedback Questionnaire at the end of service for all clients where the users of our services are invited to share their opinions and feelings about their experience with the Agency.
